Submission of Evidence The following points are for the guidance of those who are able and who agree to act as witnesses in the legal prosecution of Massoud Rajavi. A witness statement is submitted in the first instance to the police so that it is possible to determine what kind of evidence can be taken from the individual. This will give an indication as to whether the person will be required to attend court to give evidence. If you wish to submit a witness statement please read the following points first. The statement does not need to be in detail at this stage. It should be around 500 words, and can be in your own or your adopted language. The statement should contain information as defined below where relevant. · name and contact details (address, email, phone etc)· sex male/female· date of birth· Type of witness - victim, relative of victim, eye witness· political affiliation at time of incident (indicate membership of political organisation and position) and current political affiliation· profession/status at time of incident - (ie: member of a resistance army? civilian?)· date & location of incident(s) which have taken place within the past eight years· detained - length of detention and brief details of charges brought and/or of trial if any· brief details of incident(s) of violence against person or property, description of the injuries suffered (physical and psychological if any) and what medical assistance was given if any.· alleged perpetrator - actual name and/or description of person if name not known. (indicate where name used is not official name - ie; nickname)· consent to share information with other organisations and/or authorities (police & governmental bodies)· Where a witness statement has been taken by a third party. Name and address/contact details of the person taking the statement, date statement taken and signature.
